- A statement under penalty of perjury that removal was due to mistake or misidentification.
- Name, address, phone number, and email address.
- Consent to the jurisdiction of the applicable Federal District Court.
- Acceptance of service of process from the claimant (or claimant agent).
- Physical or electronic signature of the subscriber.
Repeat Infringers
Where appropriate and consistent with law, repeat infringers may have content access terminated or partner relationships suspended.
Misrepresentation
Knowingly misrepresenting infringement (or wrongful removal) may result in legal liability under applicable law.
